Vera Whole Health will use Health123’s coach-patient collaboration tools to help patients make positive, healthy behavior change

Vera Whole Health offers onsite and near-site health care to employers, including primary care, urgent care and health coaching. The company’s goal is to help people establish healthy lifestyles and take preventive measures to avoid chronic illness, which ultimately leads to reduced total cost of care.

As part of the collaboration, Health123 will provide Vera Whole Health with online tools and data to help patients track and manage their health. Tools include Health123’s dashboards, online health assessments, mobile app, health and fitness tips and more. Not only will these tools be available to patients, but Vera Whole Health coaches will have a completely connected portal with their own toolset and analytics to monitor patient outcomes and further support their healthy lifestyle changes.
