Oak Street Health and Tabula Rasa Healthcare Aim to Improve Patient Outcomes Through Optimization of Medication Regimens

Program's goal is to improve the safety of patient medication regimens and patient outcomes using Tabula Rasa's novel and proprietary MRM technolgy platform in tandem with the automated, robotic dispensary of TRHC's CareKinesis Pharmacy.

Moorestown, NJ, October 15, 2015 --(PR.com)-- Oak Street Health, a Chicago-based network of primary care doctor’s offices specializing in the healthcare of older adults, and Tabula Rasa HealthCare, Inc., a leader in providing patient-specific, data-driven technology and solutions for health care organizations, announced a new pilot program to improve patient outcomes for Oak Street Health (OSH) patients. This new Medication Risk Mitigation™ (MRM) pilot will be launched in October at Oak Street Health’s Bronzeville Clinic and will focus on OSH’s highest-risk patients.

According to Dr. Rath Chau of one of the neighborhood clinics, “our goal is to ensure optimal therapeutic outcomes for targeted patients through improved medication use.”

The pilot program is a proof of concept for improving the safety of patient medication regimens using Tabula Rasa’s novel and proprietary MRM technology platform in tandem with the automated, robotic dispensary of Tabula Rasa’s (TRHC) CareKinesis Pharmacy.

With 4.3 billion prescriptions filled in the United States in 2014, medication treatment is the most common medical intervention. Its imprecise use represents the fourth leading cause of death and contributes to an estimated 45-50 million adverse drug events (ADEs) annually.

“The incidence of ADEs is highly correlated to the number of medications an individual is taking, as well as non-adherence to prescribed regimens,” said Tabula Rasa HealthCare CEO Calvin H. Knowlton, BscPharm, M.Div., PhD. “Working with Oak Street Health, we are looking to improve the prescribing process, increase medication safety, and reduce adverse drug events and hospitalizations for their patients.”

The program will consist of several elements including integration of OSH’s Electronic Medical Record (EMR) and TRHC’s EireneRx® Medication Risk Mitigation platform. Outcomes to be measured include quality and cost improvements during the pilot, as well as data collection on medication adherence, number of patient falls, number of ER visits and hospitalizations and more.

“We will be looking to TRHC’s specially-trained clinical pharmacists and technicians to collaboratively develop with us personalized medication regimens for each enrolled patient, monitor the ongoing medication care plan with each newly prescribed medication, and conduct care plan reconciliation,” said Dr. Chau. “We are particularly excited that TRHC’s pharmacists will employ Medication Risk Mitigation™ to assess the safety of medication regimens for each OSH patient.”
