Experience with the MACS (Minimal Access Cranial Suspension) Facelift

The MACS facelift is a short scar technique that avoids alteration of the hairline and avoids open neck incisions and the associated complications. Dr. Gray studied 178 patients over an 11-year period from 2005 through 2015. All 178 patients were outpatients and had minimal anesthesia.

The advantages of the MACS facelift surgical technique are a natural appearance, avoidance of visible signs of surgery, avoidance of complications, quick recovery and minimal scars. The technique involves no cautery, no open neck incision and no drains, which is an important feature. Patients find drains to be very uncomfortable. There is less dissection and risk to nerves and muscles.

Dr. Gray found that he obtained better face and neck contours than with traditional techniques for facelift. Specifically, Dr. Gray obtained a better definition of the neck and jaw line because the muscles are pulled superiorly reversing the effects of gravity. Other techniques pull laterally and can create a widened mouth or unnatural lines on the face. There is no need to open the neck with the MACS technique.

Patients are very happy with the shorter scars of the MACS facelift as compared to the traditional method. The MACS technique avoids alteration of the hairline and nerve injuries do not occur with this technique.

There is also minimal discomfort from the MACS facelift. Most patients require only minimal use of pain management medications. With the traditional method, there was more pain and swelling.
