Plan participants receive health program that provides personalized intervention communication.

My Wellness Alerts develops a unique, personal program utilizing health records of plan participants. Compilation of medical and pharmacy claims, plan enrollment information, and biometrics and lab values identify participants' health status as being high, moderate or low risk. Based on these risk categories, HMC reaches out to members, engaging them with targeted and personalized interventions, with the intent to improve and maintain wellness.
One component of member engagement is the My Wellness Alerts® Application provided through the Benefitfocus® Platform. The My Wellness Alerts App proactively sends "My Wellness Alerts" to plan members when preventive services are recommended or a "gap-in-care" is identified through analysis of individual members' healthcare data. This innovative solution uses health and benefits management technologies to help organizations reach wellness goals, increase member engagement, improve patient outcomes and decrease overall health costs. "My Wellness Alerts" are based on unique health data and focus only on the relevant issues pertaining to that individual. Employees are able to choose how they would prefer to receive alerts, including through a SMS text message, e-mail, telephone or mail.
