"When we first sought data on case management several years ago, we examined the field as a whole," notes Melanie Matthews, HIN executive vice president and chief operating officer. "What emerged was the practice of case managers working alongside healthcare providers in a bid to better stratify and manage high-risk, high-cost populations. Our new survey will collect metrics on this approach, including placement, responsibilities, results, ROI and more from embedded case management initiatives."

One respondent who completes the survey will be selected to receive a PDF download of HIN's best-selling special report, "Hybrid Embedded Case Management: New Model for Cross-Continuum Care Coordination," an $87 value.

HIN's 10 Questions on Embedded Case Management will assemble a comprehensive set of best practices and benchmarks on the co-located case manager approach and its impact on clinical, financial and operational metrics.
