Healthcare Publisher Analyzes Chronic Care Management Approaches

As CMS begins to reimburse physician practices for select Medicare chronic care management (CCM) services, a Healthcare Intelligence Network survey examines CCM efforts by healthcare stakeholders through January 31, 2015.

Manasquan, NJ, January 09, 2015 --(PR.com)-- Healthcare organizations are invited to describe chronic care management programs, challenges and results in a brief survey from the Healthcare Intelligence Network. Survey participation will be open through January 31, 2015.

Anticipating the rollout of CCM reimbursement and incentives from other payors in the marketplace, the healthcare publisher said metrics gleaned from the survey would assist healthcare providers in honing chronic care management approaches.

Respondents receive an executive summary of results from the survey, which collects key CCM metrics, such as targeted populations, risk stratification methods, and financial and clinical impacts.

"By reimbursing physician practices for the care of individuals with chronic disease, CMS signaled its recognition of chronic care management as a foundation of primary care," notes Melanie Matthews, HIN executive vice president and chief operating officer. "We're collecting data on existing CCM programs so that healthcare organizations may learn from peers' efforts."

One respondent who completes the survey will be selected to receive an Adobe Acrobat PDF version of "Physician Reimbursement for Chronic Care Management: Identifying New Practice Revenue Opportunities," a 25-page report valued at $95.

HIN's 10 Questions on Chronic Care Management will assemble a comprehensive set of best practices and benchmarks on care coordination for chronic illness, including the following data points:

-Prevalence of CCM programs;
