This is the third time that South Nassau Communities Hospital Home Care has been named to the HomeCare Eliteā„¢ Top 500 List of home health care providers, which was published for the first time in 2006.

This is the third time that South Nassau Home Care has been named to the list, which was published for the first time in 2006.

HomeCare Elite Top 500 is compiled by Outcome Concept Systems (OCS), Inc., the leading post-acute healthcare information company in the U.S. OCS is recognized for maintaining the nation’s most comprehensive set of measures covering clinical outcomes, financial performance, patient-satisfaction and operational efficiency for home health, hospice and private duty agencies.

South Nassau Home Health Care is licensed by the New York State Department of Health and certified to participate in Medicare, Medicaid and Blue Cross programs, as well as accredited by The Joint Commission.

The HomeCare Elite ranking lists the top 25 percent of home health agencies and programs in the U.S. according to performance measures in quality, improvement and financial performance. The data used for this ranking was compiled from publicly available information from Home Health Compare and the Center for Medicare/Medicaid Services (CMS) cost reports. Three performance factors were included in the calculation of the score –quality of care, quality improvement, and financial performance.

The quality of care measure indexed each agency’s performance in the twelve public release Home Health Compare measures. The quality improvement index was based on changes in the twelve outcomes between data released in September of 2008, and in September of 2009. The financial performance measure represented agencies’ indexed profit margin from the 2008 CMS cost reports.

Each agency’s performance in each of the three domains was compared against the performance of other organizations in their state and in the country. The three domains were combined into one overall score, using a weighting methodology that gives the greatest importance to quality of care. The top 25 percent of agencies in each region, both hospital-based and freestanding, were identified and listed on the national report.
