Portsmouth, NH, May 20, 2016 --( ChartaCloud Technologies LLC (“ChartaCloud”) announced today that its technology enabled health care business unit, ChartaCares, has launched an extension to its tablet based MyHomeReach unified care recipient/care giver system called the MyHomeReach Services Center. The new extension enables MyHomeReach users to have unified access to the health care industry’s leading senior service and support organizations, all from one unified tablet interface, eliminating the need for Seniors to ‘jump from site to site’ to secure access.

MyHomeReach is a tablet-based, unified health application that enables care recipients, care givers, family members, community service organizations and volunteers to connect with each other to enhance the security, confidence and quality of care being experienced by seniors who are under care or aging at home.

The MyHomeReach application facilities communicate daily regarding a care recipients' condition. The service prompts care recipients to respond to questions related to their care plans and medication adherence, and provides the ability to capture and trend health monitoring vital signs all via a ‘one touch’ button interface designed for seniors.
