PHA Releases Whitepaper on Framework to Optimize Senior Care

Washington, DC, October 28, 2017 --(PR.com)-- The Population Health Alliance is pleased to release their industry thought leaders health brief entitled, Population Health Management: Our Framework for Optimizing Senior Health Across the Continuum. “This work reflects the strong commitment to senior health of our multi-stakeholder alliance and the promotion of health and wellbeing along with the avoidance of unnecessary complications and unwanted institutionalization,” said Rose Maljanian Chairman & CEO HealthCAWS, Inc. and the Population Health Alliance Board Chair. The sheer volume of older Americans aging into Medicare daily makes the optimization of senior health both a wellbeing and economic concern facing all citizens, added Maljanian.

In addition to the release of the brief, Ms. Maljanian will be joined by lead author and geriatrician, Dr. David Chess, Co-Founder and Chief Medical Officer of TripleCare in a webinar to be held November 16 at 3-4pm Eastern Time. The webinar will include an overview of the paper as well as time to engage with the audience in a question and answers session to discuss strategies to improve health and care for seniors across the continuum from wellness to end of life.
