Wellbridge Health Adds Director of Community Health to Growing Team of Wellness Experts

Pittsburgh, PA, March 07, 2017 --(PR.com)-- Wellbridge Health (WBH) adds healthcare expertise and passion with Rachel Hersh, RN, BSN, the new Director of Community Health. Rachel’s role at Wellbridge is a central one in the organization. Ms. Hersh will oversee the newly created Community Health Worker team, focusing on bringing socioeconomic support to patients in the community.

The Community Health Worker (CHW) Program is designed to engage health plan members/patients in the communities where they live. Benefits of the CHW Program range from assisting members/patients in keeping appointments with their primary care physician to educating members about how to use health care and social service systems to reduce barriers of health care access and improve their quality of living.

When asked what drew her to join the WBH team, Ms. Hersh replied “I was looking for an opportunity to impact community health and health literacy on a large, tangible scale. I believe there is so much value in the way we look at the experience of disease in a very holistic and innovative way.” Wellbridge implements an interdisciplinary team to optimally impact members medically and psychosocially with a team of social workers that work with members to teach a variety of self-care and communication skills to increase their health literacy thresholds so that they can become more involved in their own care.

Prior to joining the Wellbridge team, Ms. Hersh was a staff nurse in an acute stroke unit and spent years in the dual role of ambulatory care nurse and associate clinical manager at a federally qualified health center in Pittsburgh, PA. On her career path decisions, Ms. Hersh says “It was in the clinic environment that I fell in love with caring for the underserved population and being a nurse. I have served patients in every stage of the health care continuum,” she says, “this has made all the difference when thinking through solutions to positively impact members with chronic conditions.”
